Reporting EHS Incidents, Injuries, and Illnesses
Rensselaer is dedicated to providing and maintaining a safe campus work environment. EHS incidents that occur on during the course of employment with Rensselaer need to be reported to supervision in a timely manner for required notifications, recordkeeping, and response actions and repairs to take place.
An EHS incident is an event that results in an occupational injury, illness, or an exposure to an individual, a near miss, or equipment damage that occurs in Rensselaer owned facilities for institute employees. A near miss is an unplanned event that did not result in an injury, illness, exposure, or cause extensive damage, but had the potential to do so. Examples of EHS incidents include, but are not limited to, injuries, chemical spills and exposures, fires, and smoke events.
An institute employee is an individual that is compensated for their work at Rensselaer, including student employees.
Trend Analysis
The EHS office is responsible for the management of Rensselaer’s Occupational Safety and Health Administration (OSHA) 300 injury and illness log. The OSHA 300 log and trend analysis of reported EHS incidents that occur on campus provide statistical data that is useful in identifying common trends for prevention of future occupational injuries and illnesses for our campus community.
EHS Incident Reporting Procedures
An EHS incident is an event that results in an injury, illness, or exposure to an individual, damage to equipment, or is considered a near miss.
- Contact Public Safety at (518) 276-6611 or 911 if you are in need of emergency assistance
- Public Safety is available 24 hours a day
- All EHS Incidents that occur involving employees, student employees, visitors, and contractors on institute properties must be reported to EHS by completing an EHS Incident Report
- If an EHS incident involves an injury, illness or exposure, employees (including student employees) must report the incident to their manager/supervisor immediately. The incident must be reported to Human Resources within 24 hours.

- Complete an EHS Incident Report
- Following the submission of the EHS Incident Report form, please report the incident to Human Resources within 24 hours, which is crucial for meeting notification requirements and time sensitive follow-up actions
